Having acquainted myself with the rules, I have completed this application and submit it for my entry into the tournament. In signing this application, I hereby agree to be bound by and comply with all tournament rules and regulations. I expressly assume all risks associated with the tournament and I hereby release KBF the host, sponsors and tournament officials from all claims or injury and/or damage incurred in connection with this tournament. I further understand and agree that the Tournament Director reserves the right to reject my application for any good reason. If rejected, refund of entry fee will be made. I am currently a member in good standing of the KBF, TBF, & FLW. If using a boat in this tournament, I do have liability insurance.

Check-in time: 3:00 PM until 4:45 PM                          Meeting will start at 5PM local Time

Upon submitting this application, the Entry will be held by KBF for 10 days from date of submission, waiting receipt of payment. Upon receipt of payment, the entry will be processed.

Entry Form Cutoff Date:  15 days prior to tournament date   

$25 late fee for entries received after cutoff date. Angler/Co-Angler changes after cutoff - $25 late fee
